ON CERTAIN VEHICLES, A LOCKNUT ON A BOLT HOLDING THE BRAKE PEDAL LEVER TO THE BRAKE PEDAL BRACKET MAY NOT HAVE BEEN TIGHTENED ENOUGH TO ENGAGE THE LOCKING MECHANISM OF THE NUT. THIS NUT COULD VIBRATE LOOSE, AND THE BRAKE PEDAL WOULD NO LONGER FUNCTION.
THE ENGINE BOX ON SOME UNITS MAY HAVE BEEN INCORRECTLY MANUFACTURED, CAUSING IT TO PROTRUDE PAST THE TOE BOARD FLANGE. THIS PROTRUSION COULD HINDER THE MOVEMENT OF THE DRIVERS RIGHT FOOT FROM THE ACCELERATOR PEDAL TO THE BRAKE.

THE CABLE ACTUATING EMERGENCEY DOOR RELEASE MAY NOT FUNCTION PROPERLY DUE TO A STAKED KEY IN THE COLLAR OF THE EMERGENCY RELEASE CABLE. THIS CONDITION MAY CAUSE THE RELEASE CABLE TO BIND IF THE T HANDLE IS ROTATED AS IT IS BEING PULLED.
